An evaluation is an online training feedback form associated with learning activities.  This is a multi-step process that requires you to create an evaluation with questions and then assign it to relevant learning activities.

You can use the Evaluations page to create and modify evaluations and their questions.  Once you do, you can then assign evaluations to learning activities.

  1. Click Admin

  2. Click Learning Management

  3. Click Activity Management

  4. Click Evaluations

  5. Click New

  6. Input the Name

  7. Check the Active checkbox

  8. Verify the Domain

  9. Input a Description

  10. Add Instructions

  11. Scroll down to the bottom and click OK (not shown)

  12. Click Questions

  13. Click New. Note:  There are 3 question types you can select from here.  Comment, Multiple Choice, and Multiple Select.  This job aid will show how to create each type.

Comment Evaluation Questions

  1. At the bottom set the Type to Comment (this is necessary to trigger the form set up which is dependent on the question type)

  2. Fill in the Name

  3. Add a Description

  4. Input your Question Text

  5. Select Required if you wish

  6. Click OK. This will return you to the Questions page.

  7. Click New (not shown) to add another question

Multiple Choice Evaluation Questions

  1. At the bottom set the Type to Multiple Choice.

  2. Fill in the Name

  3. Add a Description

  4. Input your Question Text

  5. Select Required if you wish

  6. Add in your answers in the Choice fields (there are ten of them)

  7. Click OK. This will return you to the Questions page.

  8. Click New (not shown) to add another question

Multiple Select Questions

  1. At the bottom set the Type to Multiple Select

  2. Fill in the Name

  3. Add a Description

  4. Input your Question Text

  5. Select Required if you wish

  6. Add in your answers in the Choice fields (there are ten of them)

  7. Click OK. This will return you to the Questions page.

  8. Navigate back to Evaluations and Click the checkbox next to it to select the evaluation you just created.

  9. Under Task Select Activate Evaluation.

  10. Click the Blue Arrow

You are now ready to assign the evaluation.

Adding the evaluation to a Learning Activity

  1. Navigate to the Activity Management Page

  2. Locate the Learning Activity you wish to attach the Evaluation and click Edit

  3. Click Send to Stage (not shown)

  4. Click Optional (not shown)

  5. Select Evaluations which is on the Registration menu (not shown). This will open the Evaluations dialog box.

  6. Click Add. This will open the Add Evaluations dialog box

  7. Select the Evaluation you created

  8. Click Next. This opens the Notifications Trigger dialog box.

  9. Set the Notification Trigger. Options are:
    • Activity Start
    • User Complete
    • User Registration
    • User Launch

  10. Input the Trigger Offset

  11. Input Days to Complete the evaluation

  12. Set an Activity Evaluation date (if desired)

  13. Click OK

  14. Click Validate for Production (not shown)

  15. Click Move to Production
