Apple iPhones and iPads are the DES mobile device standard. If you have a reasonable accommodation and need an android device, your manager/supervisor will need to submit a ticket to DES IT Support with the business justification. DES IT Support will evaluate the request.

If you would like to request a mobile device, follow the directions below.

  1. Read the Guidance articles listed below so you understand the difference between state issued and personal devices.

  2. Fill out the online form located here: DES-Mobile_Device-Request-Form

  3. Once you have completed the form and certification page, the form will automatically be routed to your manager/supervisor for approval or rejection.

  4. If your request is approved, a ticket will be automatically submitted to DES IT Support on your behalf. There are no extra steps for you take.

  5. DES IT Support will process your request and contact you if they have any questions. It usually takes 3-5 days to receive your new Mobile Phone.

 

Navigating the Mobile Device Request Form

The instructions below will walk you through the request process for a state issued mobile device or the annual renewal process. 

Request a State Issued Mobile Device

  1. Launch the MDRF DES-Mobile_Device-Request-Form.



  2. Choose New Mobile Device Request from the drop down under Request Type.



  3. Choose State Issued Device from the Device Type drop down.



  4. Choose the Business Justification that relates to your need for a state issued mobile device.

  5. Enter your program Budget Code for ordering a state issued mobile device. This is required before you can proceed with ordering a device.



  6. Once all the required fields are completed, the Submit button illuminate at the bottom. Click Submit to move to the next section.
  7. In the Request Certification section, you will need to read and toggle each button to Yes. Please read this section so you are aware of the policies and understand your mobile device can be wiped at any time and is a part of DES Mobile Device Management and will be managed by DES IT Support. Once you have completed this section, the Certify button will illuminate.



  8. Once you click Certify, you are done. The request will be automatically sent to your manager/supervisor for approval. DES IT Support will reach out to you when your mobile device is ready for pickup. 

Submitting the Annual Renewal for your State Issued Mobile Device

All employees who are assigned a state issued mobile device or receive a stipend, just complete the annual renewal form. Below are the instructions for filling out the annual renewal.

The instructions for the annual renewal are the same as above. The only step that will change is step number 2.

  1. Launch the MDRF DES-Mobile_Device-Request-Form.

  2. Choose Annual Mobile Device Renewal from the drop down under Request Type.



  3. Continue on with steps 3 though 8 above.

 

Request to use your Personal Device and Receive a Stipend (not recommended)

  1. Launch the MDRF DES-Mobile_Device-Request-Form.



  2. Choose New Mobile Device Request from the drop down under Request Type.



  3. Choose Personal Device from the Device Type drop down.



  4. Choose a Device Plan from the Personal Device Plan (Stipend) menu.



  5. Enter the phone number of your personal device in the Personal Device Number box.



  6. Choose the Business Justification that relates to your need.

  7. Enter your program Budget Code. This is required before you can proceed.



  8. Once all the required fields are completed, the Submit button illuminate at the bottom. Click Submit to move to the next section.
