This handout will walk you through the process of completing the new HR Workflow forms for employees who are new to DES or leaving DES.
Submitting a PAR Request (Do Not Use if hiring a Contractor):
- Start by visiting https://apps.des.wa.gov/HRWorkflow.
- Scroll down to Forms, click the down arrow and choose New PAR Wizard.
- Under Action Type, choose New Hire. Continue to fill out the entire “New Personnel Action Request” section. Submit the Workflow for approval.
- Once the new PAR is approved, go back to your Personal Dashboard in HR Document Workflow and click the dropdown under the Waiting for Other (1 Items) area.
- On the right hand side, you will see a box under IT Request called “New Employee IT Request”. Click on the button to fill out the IT request for the New Employee.
- Some of the fields will be auto filled from the PAR form. Any field with a * after the title is a required field and you will not be able to submit the request without filling in the information.
- Anytime you see the Search box above a field, click the box to search for the Supervisor/User from Active Directory.
- Once you have completed the Exit Employee IT Request form, click Submit and the request will be sent to DES IT Support 4U. You will receive a confirmation email from DES IT Support 4U.
Additional Tips:
- Be sure to always include an Employee to Mimic as well as a cost code for any non-standard equipment or software the new employee will need. Including all of the details on the IT Employee Request form will insure that the New Employee has everything they need on their first day of employment with DES.
- Make sure and submit the IT Request form at least 2 weeks prior to the New Employee starting to ensure that all IT purchases, accounts and access are in place before the new employee starts.
Submitting a SAR Request (Do Not Use if Employee is a Contractor):
- Start by visiting https://apps.des.wa.gov/HRWorkflow.
- Scroll down to Forms, click the down arrow and choose New SAR Wizard.
- Enter the Employee Details and click the next arrow. Continue to fill out the “New Separation Action Request” section and click Finish.
- Under Form Review, click Submit.
- Go back to “My Workflow Items” and click on the arrow at the far right of the page.
- On the right hand side, you will see a box under IT Request called “Exit Employee IT Request”. Click on the button to fill out the IT request for the Exit Employee.
- Some of the fields will be auto filled from the SAR form. Any field with a * after the title is a required field and you will not be able to submit the request without filling in the information.
- Anytime you see the Search box above a field, click the box to search for the Supervisor/User from Active Directory.
- Once you have completed the “Exit Employee IT Request” form, click Submit and the request will be sent to DES IT Support 4U. You will receive a confirmation email from DES IT Support 4U.
